San Diego PACE is upgrading our computer systems to provide better care and an improved experience for you. We appreciate your patience as you may notice delays at our call center or during your clinic visits.
Enrolling in San Diego PACE means full coverage. The process for enrollment in San Diego PACE starts with an initial home visit and a basic clinical assessment. During this visit, you will have an opportunity to speak with one of our Registered Nurses and Social Workers about your specific health needs. Upon enrollment, PACE participants will receive all necessary medical services from San Diego PACE as approved by their clinical team.

Once we receive your contact information, an enrollment specialist will contact you directly to discuss your situation and schedule an In-Home Assessment. This assessment will help us certify that you are eligible for our services.
Upon enrollment, PACE participants agree to receive all services from San Diego PACE.
If you are making a referral for a family member or a friend, please provide their name and contact information.
Who qualifies for San Diego PACE?
There are no financial eligibility requirements to become a PACE participant. In order to be eligible for this program you must:
